Programs
The
Churchill Program
The
Churchill Program operates in a 16-bed acute
residential setting. The patient population
includes residents requiring 24-hour nursing,
residents in the early stages of the post-acute
phase of recovery, and residents requiring
evaluation for appropriate neurobehavioral
treatment. Residents may be dependent in all
activities of daily living. Treatment along the
post-acute continuum of care may extend from
short-term (30-180 days) treatment, to long-term
care (greater than 180 days) for persons requiring
care for ongoing medical/neurobehavioral
interventions.
The goals of the Churchill Program are to deliver
effective and efficient treatment for improving neuropsychiatric status, behavioral function, and
for promoting participation in daily meaningful
productive activities. Meeting these goals allows
residents to safely transfer home or to other less
restrictive environments. For persons requiring
long-term care, the goals include maintenance of
the highest level of independent functioning
achieved, prevention of recidivism to acute care,
and promotion of quality of life through
participation in meaningful daily and leisure
activities.

Admission criteria
Most admissions to Touchtone are made to the
Churchill. Level of care indicators for
admissions to Churchill program may include:

Must be at least 16.5 years of age
-
Nursing intervention requiring at least 30
minutes per day, including medication
administration and neuromedical baseline
observations.

Cases requiring formal consultation with
attending physician equal to or greater than
two times per month.

Formal rehabilitation therapies necessary
upon admission, or greater than or equal to
two concurrent therapies in two or more
discipline areas.

Part-time to full time supervision
requirements.

May require total assistance on all measures
of self-care, physical therapy, speech
therapy, occupational therapy, transfers and
locomotion.
Residents may be transferred to the
Somerset,
Genesis or
Community-Based Living Rehabilitation programs
are based on the demonstrated need for another
level of care in accordance with
admission criteria for those programs.
