Programs
Community-Based Living
Our Community-Based Living Program provides an
essential treatment component for the transition
of brain injury survivors back into the community.
Programs and services offered are designed to lead
either to the maintenance of the maximum
achievable independence level within a supervised
setting or total independence within the
community. Residents may enter the program as the
final phase of Touchstone’s continuum of care or
by transferring from another care setting.
Supervised living program
The Supervised Living Program consists of
individual homes in community neighborhoods. Two
levels of programming are offered depending on the
individual's level of neurological impairment and
physical needs. The first level provides a
home-like environment for residents continuing to
display cognitive and/or physical deficits, but no
longer displaying significant behavioral difficulties
on a regular basis. These clients focus on the
completion of activities of daily living, such as
menu and leisure skills planning, weekly budgeting
and medication administration.
The
second option offers the setting and life-skills
components similar to the first level, combined
with transitional programming and continued
vocational retraining. This supervised living
program is designed for residents who plan to seek
gainful employment within the community and may
ultimately live in an independent setting. Each
home offers continuous 24-hour care with
assistance for activities of daily living to the
resident as needed.
Supported living program
The Supported Living
Program also consists of individual homes in community
neighborhoods, but provides a more independent
environment with staff available to the residents
on a daily basis. Residents at this level of care
address higher level activities of daily living
with minimal staff involvement. The residents are
required to be gainfully employed within the
community setting.
Residents may attain goals such as driving their
own vehicles ,once they have completed an
independent driver evaluation and have obtained a
drivers license, insurance and all necessary
permits. Residents live in the home without
regular direct staff supervision; however, staff
members assist in higher level activities of daily
living, visit on a daily basis, and are available
by phone. Residents at this level of care focus on
moving toward total independence and living
unsupervised within the community setting.
Admission criteria

At
least 18 years of age.

Known or suspected neurological or
neurobehavioral impairment.

Medically stable and not requiring routine
nursing care.

Able to self-administer medication, if
needed.

Capable of attending vocational programming
on a full time basis or maintain community-based employment.
Residents who demonstrate the need for a higher
level of supervision or care, due to a change in
physical or behavioral conditions, may be
transferred to a more appropriate level of
rehabilitation within the Touchtone continuum if necessary.
