The
Windsor Program
The
Windsor Program houses an unique environment for
adolescents, 16.5 and older, and adults with
neurological impairments of varying degrees.
Windsor is licensed by the Texas Department of
Aging and Disability Services (TDADS) with a
special certificate for managing those with memory
loss or dementia and is accredited by the
Commission on Accreditation of Rehabilitation
Facilities ( CARF ).The secured 16-bed facility is
designed to provided comprehensive
neurobehavioral, cognitive, physical , and
pre-vocational rehabilitative services at a
post-acute level of care.
The
facility offers 24-hour-a-day, seven-day-a-week
supervision by specially trained staff, Living
Skills Advisors (LSAs) and nurses. Windsor
provides structured activities in a prosthetic
environment to assist residents with achieving or
maintaining the highest level of independent
function and best possible quality of life.
Program objectives are individualized following
identification of barriers, querying desired goal
of the resident, stakeholder and family members,
performing assessments, and establishing
attainable outcomes.
Admission criteria

Must be at least 16.5 years of age.

Has a known or suspected neurological or
neurobehavioral impairment.

Must be medically stable
-
Requires full time supervision
-
May require nursing intervention at least 30
minutes per day, including medication
administration and neuromedical baseline
observations.

Requires formal consultation with attending
physician equal to or greater than two times
per month.

May require formal rehabilitation therapies upon admission, greater than or
equal to two concurrent therapies in two or
more discipline areas.

Has 24-hour awake staff supervision
requirements.

May require total assistance on all
activities of daily living.

Residents may be transferred to the
Churchill,
Somerset,
Genesis or
Community
Services provided by the Windsor Program may

Rehabilitation programs
are based on the
demonstrated need for another level of care
and in accordance with admission criteria for
those programs.
